This is a dedicated policy for consumer health data — the health-related information you share with us, mainly through the paid Operator Baseline intake. It exists as a separate policy because laws such as Washington's My Health My Data Act require one. For everything else — site analytics, email, purchases — see our general Privacy Policy. Where the two overlap, this policy governs your health data.
The short version. We are an educational performance service, not a healthcare provider, and we do not diagnose or treat. We collect health information only to build and support the report you asked us to build. We never sell it, never share it with advertisers or supplement brands, and never place your medication information into marketing or analytics tools. You can see it, withdraw your consent, or have it deleted at any time.
When you complete the Operator Baseline intake, we collect the health-related information you provide, which may include:
From the free Operator Score (the short assessment), we may also derive a general indicator that you do or do not take medications (a yes/no category — never the names of your medications) together with your overall score and weakest pillar. We do not ask you for diagnoses, lab results, or clinical records, and we do not want them.
We collect this information directly from you, when you fill out the intake questionnaire or the free assessment. We do not buy, rent, or obtain your health data from third parties, data brokers, or other sources.
We do not use your health data for advertising, and we do not use it to build marketing profiles.
Within Elevare Performance, your consumer health data is accessible only to Josh DeLucia, who personally builds your report, and it is stored on access-controlled systems we operate. A small number of service providers necessarily process it to deliver the service — most notably our form host (Netlify), which stores your submission until we export it to our local store and delete it there; these providers are described under "Who we share it with" below and are not permitted to use your data for their own purposes. We do not sell your consumer health data, and we do not share it with advertisers, supplement companies, or data brokers. A small number of service providers help us operate, and they receive only what they need to perform their function and are not permitted to use your data for their own purposes:
We do not sell consumer health data, and we would not do so without first obtaining your separate, valid written authorization. We have no plans to sell it.
Consistent with the Washington My Health My Data Act and similar consumer-health-privacy laws, you have the right to:
To exercise any of these, email josh@elevareperformance.ai. We will respond and act as required by law. Withdrawing consent or requesting deletion will not undo anything we already completed before your request, but we will stop and delete going forward. If we deny a request, you may reply to appeal it and we will review the appeal.
If you opt in to the optional method-improvement use, we may keep a de-identified version of your intake and report — with your name and identifying details removed. We maintain that information only in de-identified form, do not attempt to re-identify you, and use it solely to improve how we build reports.
We keep your consumer health data for as long as needed to deliver and support your report and your tracking window, and then we delete it — unless you ask us to delete it sooner, you have opted in to de-identified retention, or the law requires us to keep it longer.
Our services are intended for adults. We do not knowingly collect consumer health data from anyone under 18. If you believe someone under 18 has provided us health data, email us and we will delete it.
If we change how we handle consumer health data, we will post the updated policy here with a new effective date.
